Q1 - Name

Q2 - Contact number

Q3 - Suburb & State

Q4 - Size of floor to be laid (approximate size is fine)

Q5 - Old or new slab (pictures are great!)

Q6 - If your floor is in your home, office or warehouse

Q7 - Approximate start date

Q8 - Inclusion of any plans (if applicable)
