Support at Home 2025 Transition Series- Week 1

A major shift is coming to aged care. From 1 July 2025, the Australian Government will launch the Support at Home program- a single, streamlined model that replaces the current Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), and other home care services.
