Support at Home: Phase-by-Phase Transition Plan by CareVision.

The transition to Support at Home isn't happening all at once- it's being rolled out in five structured phases to help aged care providers prepare, adapt, and succeed. Each phase includes critical system, process, and product changes. With CareVision guiding you every step of the way, you can confidently get ahead of the changes before July 2025.

Phase 1: February-March 2025

The groundwork begins. This phase focuses on documentation, requirement planning, and consultations with clients. Providers are encouraged to map out their services and service categories. CareVision's tools are available to support data cleanup and setup.

Phase 2: April-May 2025

Funding transformations takes centre stage. Organisations must decide how to manage funding transitions, particularly around client funding. CareVision will offer an opt-in feature to auto-generate new client funding records ready for July 1st.

Phase 3: June 2025

With the go-live date approaching, this phase is all about training and preparation. Staff will be introduced to new claiming processes and educated on system changes to ensure operational readiness.

Phase 4: July 2025

Go live. The core Support at Home model is activated. Essential features will be launched, forming the minimum viable operating model. Any features not deemed critical will follow in later updates.

Phase 5: Post-July 2025

Refinement and enhancement. This final phase includes new features such as automation of client verification with Services Australia and improvements based on feedback from the initial rollout. CareVision will conduct bi-weekly check-ins with stakeholders for around six months to ensure a smooth and supported transition.
