Adapting to the New Support at Home Program with CareVision.

To keep up and prepare for Australia's evolving aged care system, CareVision will be transitioning from Home Care Packages to the new Support at Home Program, now set to commence on 1 November 2025, following a government-announced delay.

Transitioning to the Support at Home Program will be easier if you use CareVision's recommended service list. However, if you are not yet using this, the transition may require you to do the following:

• Grandfathering outdated services
• Adding the new service programs and categories
• Use of the upcoming Care Plan Audit Tool

Don't worry, as CareVision will be there to guide you through the entire process. For legacy services, the different service migration options you may choose from include:

• Renaming existing services
• Selecting one service to keep
• Suspending unused services
• Creating entirely new services

CareVision is developing two helpful tools that can further assist you with your transition from Home Care Packages to the Support at Home Program.

1. Care Plan Audit Tool - This tool will be available soon and will help you in identifying all the client care plans that use specific services, which need updating
2. Migration Tool - This tool is specifically designed for bulk migration and is ideal for organisations with a large number of clients.

Take note that the Support at Home Program will also have a few funding considerations:

• A "use it or lose it" funding model that has quarterly budgets
• Limited carryover funds that are possible between quarters (10% or $1,000, whichever is greater)
• Carryover funds will not be calculated until 60 days into the new quarter
